Social Media Release Form

This Media Release Form is effective (Date Selected Below), by (NAME TYPED BELOW) who acknowledges and agrees to the terms below:

The Party grants permission to Bandits Hockey Club Inc. To use their photos/videos in Bandits Hockey Club Inc. Publications, both print and digital forms, including the company’s website, newsletters, emails, social media posts, videos, brochures, and advertisements.

The Party consents to the use of their photos/videos by any third party, such as affiliated sponsors, in whole or in part, in all forms and media, for distribution to the public for purposes of publicity and promotion of the Company.

In giving consent, the Party releases the Company and all third parties from liability for any violation of any personal or proprietary right the Party may have in connection with any sale, reproduction, or use of their photographs.

The Party is aware that commentary is satirical and should not be considered as fully truthful.

The Party certifies that they are 18 years of age or older. If younger than 18, Legal Guardian consent and signature are required.
